Clinical Preprocessing and Informative Missingness Pipeline

Welcome to the 24th installment of the Maeumjigi Capstone Project Dev Log.

Following our successful Semester 1 final defense and dual-layer safety wrapper deployment in June, our team officially kicked off Semester 2 (Fall 2026). In the first semester, our primary engineering focus centered on large language model (LLM) fine-tuning (Llama 3.1 8B, Qwen 2.5 32B/7B), synthetic dialogue generation, and prompt safety guardrails.

However, in clinical psychology and psychiatric triage, language is only half of the diagnostic puzzle. Patient risk escalates across time through longitudinal numerical markers: self-reported depression severity (PHQ-9), generalized anxiety (GAD-7), perceived stress, and insomnia scores.

Today's dev log documents our architectural breakthrough in clinical data engineering: moving away from naive imputation methods and engineering a robust 14-Day Last Observation Carried Forward (LOCF) pipeline equipped with Informative Missingness (Missing Not at Random / MNAR) Indicators.

1. The Clinical Problem: Why Naive Imputation Fails in Psychiatry

In standard machine learning tabular pipelines, engineers frequently fill missing survey rows using global mean substitution, k-nearest neighbors (KNN), or forward-filling across infinite time spans.

In mental health crisis detection, these naive assumptions introduce fatal distortions:

flowchart TD
subgraph Naive_Imputation_Failure ["The Flawed Baseline: Infinite Forward Fill / Mean Substitution"]
A1["Patient takes PHQ-9 survey (Score: 24 - Severe Crisis)"] --> A2["Patient goes silent for 90 days (Severe withdrawal)"]
A2 --> A3["Naive Model: Carries forward score 24 for 3 full months"]
A3 --> A4["False Alarm: Alerts crisis center on Day 85 despite no recent signal"]

B1["Patient enters depressive stupor; skips survey entirely"] --> B2["Naive Imputation: Fills with population mean (Score: 8 - Mild)"]
B2 --> B3["Fatal Masking: Misses active suicide risk because missingness was ignored"]
end

subgraph Clinical_MNAR_Solution ["Maeumjigi Architecture: 14-Day LOCF + Missingness Indicator"]
C1["Patient Score Recorded on Day 0"] --> C2["Bounded 14-Day LOCF Window"]
C2 -->|Days 1-14| C3["Maintain valid clinical state with decaying recency weight"]
C2 -->|Day 15+| C4["Score expires into null; Missingness Indicator Flag set to 1"]
C4 --> C5["Model explicitly learns that non-response is a predictive risk feature (MNAR)"]
end

In psychiatric cohorts, missing data is rarely Missing Completely at Random (MCAR). A patient failing to submit their daily mental health assessment is often exhibiting behavioral avoidance, severe psychomotor retardation, or anhedonia—hallmark symptoms of escalating depression. Missingness itself is an informative diagnostic signal (Missing Not at Random / MNAR).

2. Multi-Scale Normalization Architecture

Our dataset aggregates four heterogeneous psychological psychometric scales, each with distinct scoring ceilings and clinical cutoffs:

Clinical ScaleRaw Score RangeClinical CutoffTarget Clinical DomainNormalization Strategy
PHQ-9 (Patient Health Questionnaire)0 – 2710+ (Moderate), 20+ (Severe)Major Depressive Disorder (MDD)Min-Max scaling to [0, 1] per scale
GAD-7 (Generalized Anxiety Disorder)0 – 2110+ (Moderate), 15+ (Severe)Panic & Generalized AnxietyMin-Max scaling to [0, 1] per scale
PSS (Perceived Stress Scale)0 – 4027+ (High Stress)Chronic Psychosocial StressMin-Max scaling to [0, 1] per scale
ISI (Insomnia Severity Index)0 – 2815+ (Clinical Insomnia)Sleep Disturbance & Circadian RhythmMin-Max scaling to [0, 1] per scale

The Architectural Fallacy of Single-Index Compression

Earlier student prototypes attempted to sum all four raw scores into a single aggregate index between 0 and 1.

Our clinical advisory review identified why this destroys diagnostic power: an individual with extreme acute anxiety (GAD-7: 21) but low depression (PHQ-9: 2) would receive the exact same aggregate score as a chronically depressed individual with zero anxiety. Psychiatric interventions differ completely for panic vs. vegetative depression. Therefore, our pipeline strictly preserves four independent input channels.

3. Engineering the 14-Day LOCF with MNAR Indicator

To balance recency validity with informative non-response, we designed the following temporal state machine in Python:

# clinical_preprocessor.py - Longitudinal Clinical Preprocessing Engine
import pandas as pd
import numpy as np

SCALE_BOUNDS = {
"phq9": (0, 27),
"gad7": (0, 21),
"pss": (0, 40),
"isi": (0, 28)
}

def process_patient_timeline(df: pd.DataFrame, max_locf_days: int = 14) -> pd.DataFrame:
"""
Applies bounded 14-day LOCF imputation while generating
explicit binary Missingness Indicators (MNAR) for downstream neural layers.
"""
df = df.sort_values("timestamp").copy()
processed_cols = {}

for scale, (min_val, max_val) in SCALE_BOUNDS.items():
val_col = f"{scale}_score"

# 1. Min-Max normalization preserving clinical zero
norm_series = (df[val_col] - min_val) / (max_val - min_val)

# 2. Bounded Last Observation Carried Forward (LOCF)
locf_series = norm_series.ffill(limit=max_locf_days)

# 3. Missingness Indicator: 1 if unobserved (expired or never taken), 0 if observed
missing_indicator = locf_series.isna().astype(np.float32)

# 4. Fill residual nulls with 0.0 (safe because missing_indicator tells the model it is unobserved)
final_scores = locf_series.fillna(0.0)

processed_cols[f"{scale}_norm"] = final_scores
processed_cols[f"{scale}_missing"] = missing_indicator

return pd.DataFrame(processed_cols, index=df.index)

Through this formulation:

  1. If a patient submitted a survey 5 days ago, the model receives their normalized score, with missing_indicator = 0.0.
  2. If more than 14 days elapse without response, the feature transitions to missing_indicator = 1.0, signaling chronic survey non-compliance to the risk assessment classifier.

4. Fusing Tabular Longitudinal Features with Conversational LLMs

In our final production architecture for Semester 2, these processed numerical vectors do not exist in isolation. They form a Multi-Modal Cross-Attention Fusion Bridge with the conversational token representations emitted by our fine-tuned Qwen 2.5 engine:

flowchart LR
subgraph Numerical_Pipeline ["Stream A: Longitudinal Clinical Vector"]
A1[14-Day Normalized Scores: PHQ9, GAD7, PSS, ISI]
A2[Binary Missingness Indicators: MNAR Flags]
A1 & A2 --> A3[Temporal Dense Projection Layer (128-d)]
end

subgraph Dialogue_Pipeline ["Stream B: Multi-Turn Conversation"]
B1[Real-Time Chat Turn: 'I cannot sleep and feel hopeless']
B1 --> B2[Fine-Tuned LLM Hidden States (4096-d)]
end

subgraph Multimodal_Fusion ["Dual-Stream Cross-Attention"]
A3 --> C[Gated Multimodal Fusion Unit]
B2 --> C
C --> D[Ordinal Risk Staging: Low, Moderate, High, Imminent Crisis]
end

By presenting both the patient's immediate verbal distress and their 14-day psychometric trajectory, the system eliminates both false positives (e.g., hyperbole without underlying clinical depression) and false negatives (e.g., smiling depression where severe survey scores contradict calm conversational tone).

5. Day 24 Key Achievements & Next Steps

  1. Informative Missingness Solved: Formulated a clinically grounded 14-day LOCF imputation model that treats survey absence as an active MNAR predictive feature.
  2. Preserved Multi-Scale Specificity: Rejected naive single-score compression, maintaining 4 independent channels for depression, anxiety, stress, and insomnia.
  3. Faculty Review Validation: Submitted the technical blueprint (문의_위기감지모델_숫자입력_전처리.docx) to our advising professor, receiving unanimous validation for rigorous clinical methodology.

In Dev Log #25, we dive into training our Ordinal Logistic Regression & Gated Multimodal Classifier to benchmark risk triage accuracy against standardized psychiatric evaluation sets!

📌 3–5 Line Executive Summary

  1. Clinical Data Engineering Breakthrough: Transitioned Semester 2 capstone focus from pure NLP to multi-modal psychiatric triage, preprocessing 4 distinct psychometric scales (PHQ-9, GAD-7, PSS, ISI).
  2. Informative Missingness (MNAR) Treatment: Modeled patient survey non-response as a predictive clinical signal rather than random noise, replacing naive imputation with bounded 14-day LOCF.
  3. Multi-Scale Channel Independence: Preserved 4 dedicated feature channels under Min-Max scaling, eliminating diagnostic confusion caused by single-score averaging.
  4. Multimodal Fusion Architecture: Integrated longitudinal numerical features with conversational LLM embeddings to dramatically reduce both false positive and false negative suicide crisis alerts.
